Registration form for non-regular student

Diploma or certification of completion of college degree (original and copy);
Academic transcript (original and copy);
One (1) 3 x 4 (recent) photograph identified on the back with the student’s name;
Identity card (copy);
Birth or marriage certificate (copy).
